Brain Zaps: Overlooked Symptom of Antidepressants & Withdrawal

Brain zaps is a colloquial term for one of the most troubling symptoms the people experience either while taking, or withdrawing from antidepressants and also benzodiazepines. Yet it is largely ignored in medical research.

What Are Brain Zaps? 

Medical research on brain zaps is sparse.  If you have ever looked for medical literature such as published clinical trials or studies on brain zaps you won’t find much. Perhaps this is why many prescribers may not be able to answer your questions, and some have never heard of it.

The information available on this troubling side effect indicates it is most commonly associated with either taking or coming off antidepressants as well as benzodiazepines. Information on brain zaps is sparse in the realm of published literature, and is overwhelmingly patient-forwarded. From a lone 2019 internet patient survey, the conclusion appears to be that brain zaps are real and yet have been neglected in research and physician education.

An exhaustive search found a 2005 report published in the BMJ, called “Bottled Lightning” which included a 1980s reference to “brain fizzing” associated with antidepressants. The authors of these papers called for a standardized medical definition. The term “brain shivers” was reportedly coined in 1999 and a picture based article on the subject was published briefly in the 1999 BMJ and cannot be found today. Heather Ashton mentions “electric shock” sensations in her 1999 manual listing the many antidepressant withdrawal and benzodiazepine withdrawal symptoms.1,2

How Brain Zaps Feel

Many metaphors have been used by experiencers to describe how brain zaps feel, including:

  • Buzzing sensation in the head, neck, and down the spine
  • Fizzing in the brain, or brain fizzing
  • Nerve lightning
  • Electrical jolt inside the head

These sensations can occur during a course of treatment with antidepressants or benzodiazepines, when these drugs are being reduced in dosage, when a dose is missed, or when the drug is abruptly stopped. Brain zaps can be mild or intense, and while they usually are short-lived, their unpredictability, duration, characteristics, and frequency can vary erratically, which can be quite disturbing to the experiencer.5

Other feelings often associated with the phenomenon include dizziness, lightheadedness, anxiety, nausea, headaches, vertigo, and loss of balance. Symptoms can persist while withdrawing from antidepressants, and for some, may last months or even years after stopping the drug.

Antidepressant Withdrawal and Brain Zaps: Factors that can Worsen them

Brain zaps are associated with medication withdrawal. Antidepressants and benzodiazepines are noted for brain zaps that occur during treatment and are associated with medication withdrawal. 

Other factors that have been found to intensity or worsen these sensations, include
  • Lack of sleep
  • Poor nutrition
  • Lateral (side-to-side) eye movements
  • Sudden head or neck movements
  • Poorly managed medication withdrawal
  • Skipping or missing a dose of antidepressant or benzodiazepine medication
  • A drug with a short half-life increases likelihood of brain zaps

Treatments for Drug Withdrawal Induced Brain Zaps

Reportedly, the percentage of those experiencing brain zaps associated with antidepressants and antidepressant withdrawal is overwhelmingly in the majority. Yet the factors that need addressing can vary greatly with each individual. Therefore there can be no “one-size-fits-all” approach to treatment. Treatment methods which are safe and effective are discussed below.

Well-managed Withdrawal:  The best approach is to seek proper withdrawal management help from caregivers who are familiar with the phenomenon. A very slow and steady withdrawal schedule is indicated. There are many protocols that can assist the antidepressant withdrawal process in very gentle and measured ways.

Adequate Nutrition:  Adequate nutritional intake can act as a recovery tool. Testing can clearly outline the guidelines needed to correct deficiencies or discover allergies, so that specific dietary choices can provide the best individualized support for recovery. Can neurotoxins be damaging to mental health?

Yes. The body creates natural chemicals, that act as messengers via nerve cells throughout the body. One example of how neurotoxins can damage nerve cells, is that they can attack and degrade the nerve cell’s protective myelin sheath, which can disable normal messaging in the central nervous system. Yes. Drugs including medications and recreational drugs like cannabis can damage neurons.8

Why did my doctor not warn me about brain zaps?

The medical literature is sparse concerning the phenomena associated with antidepressants, antidepressant withdrawal or withdrawal from benzos. It is quite possible your prescriber was unaware of the potential for brain zaps either while taking or withdrawing from these substances.3-5

Are brain zaps listed on all drug labels as potential adverse effects or noted as withdrawal symptoms?

Not all. Examples of SSRIs and SNRIs that do list electric shock sensations include Lexapro (escitalopram), Zoloft (sertraline), Cymbalta (duloxetine), Prozac (fluoxetine), and Paxil (paroxetine). Benzos such as Restoril (temazepam) and Serax (oxazepam) do not, despite the short half-life feature of these drugs.9-15
