What Are Brain Zaps?
Medical research on brain zaps is sparse. If you have ever looked for medical literature such as published clinical trials or studies on brain zaps you won’t find much. Perhaps this is why many prescribers may not be able to answer your questions, and some have never heard of it.
The information available on this troubling side effect indicates it is most commonly associated with either taking or coming off antidepressants as well as benzodiazepines. Information on brain zaps is sparse in the realm of published literature, and is overwhelmingly patient-forwarded. From a lone 2019 internet patient survey, the conclusion appears to be that brain zaps are real and yet have been neglected in research and physician education.
An exhaustive search found a 2005 report published in the BMJ, called “Bottled Lightning” which included a 1980s reference to “brain fizzing” associated with antidepressants. The authors of these papers called for a standardized medical definition. The term “brain shivers” was reportedly coined in 1999 and a picture based article on the subject was published briefly in the 1999 BMJ and cannot be found today. Heather Ashton mentions “electric shock” sensations in her 1999 manual listing the many antidepressant withdrawal and benzodiazepine withdrawal symptoms.1,2
How Brain Zaps Feel
Many metaphors have been used by experiencers to describe how brain zaps feel, including:
Brain shivers - Electric shock feelings
- Audible sound of electric zap in the head
- Buzzing sensation in the head, neck, and down the spine
- Fizzing in the brain, or brain fizzing
- Nerve lightning
- Electrical jolt inside the head
- Brain flips
- Brain pops
- Brain lightning
These sensations can occur during a course of treatment with antidepressants or benzodiazepines, when these drugs are being reduced in dosage, when a dose is missed, or when the drug is abruptly stopped. Brain zaps can be mild or intense, and while they usually are short-lived, their unpredictability, duration, characteristics, and frequency can vary erratically, which can be quite disturbing to the experiencer.5
Other feelings often associated with the phenomenon include dizziness, lightheadedness, anxiety, nausea, headaches, vertigo, and loss of balance. Symptoms can persist while withdrawing from antidepressants, and for some, may last months or even years after stopping the drug.
